I don't think it is mandatory. The specification states that any needed
device specific registers must be placed after the predefined header
portion. I interpret this as you mustn't put any device specific registers
in the predefined header - as you know, all the registers don't need to be
implemented.
Specification also says that configuration space is provided for enabling
basic operation and reporting severe system errors. So you should put a
device specific register in the configuration space if it is needed to
enable normal operation, otherwise it is not necessary.
Host bridge doesn't even need a configuration space.
In guest implementation, the bridge provides access to basic functions - you
can enable/disable everything, map/remap base addresses etc..
